TMF Management - Evidence That the Trial Was Conducted in Compliance

The Trial Master File is the collection of essential documents that allows the conduct of a trial and the quality of the resulting data to be evaluated, and confirms compliance with GCP and applicable regulatory requirements. ICH E6(R2) Section 8 defines the essential document categories expected before, during, and after a trial, while the industry TMF Reference Model provides the standard taxonomy most sponsors and CROs build their filing structure around. This map details the pathway from TMF structure and planning through real-time filing, completeness review, and archival.

Trial Management

Trial-level oversight documents: protocol, trial management plan, monitoring plan, and committee records.

Owner: Sponsor

Central Trial Documents

Documents applicable trial-wide: regulatory submissions, safety reports, and central lab or vendor agreements.

Owner: Sponsor / CRO

Site Management

Site-level documents: regulatory approvals, delegation logs, training records, and monitoring visit reports.

Owner: Site / Investigator

IP and Safety

Investigational product accountability records and safety reporting documentation across the trial.

Phase 1
TMF Structure and Planning
1Establish the TMF structure and filing plan
Filing structure built on a recognized taxonomy, typically the TMF Reference Model, defining zones, sections, and artifact types before the trial begins.
TMF Reference Model
↓
2Assign TMF ownership and access roles
Named TMF owner assigned with defined roles for filing, quality review, and approval, along with access controls appropriate to each role.
ICH E6(R2) Section 8
↓
3Define the essential document index
Trial-specific index built from the ICH E6(R2) essential document list, tailored to the protocol's specific vendors, committees, and regulatory pathway.
ICH E6(R2) Section 8
↓
4Set up the investigator site file structure
Site-level file structure established at each participating site, mirroring the sponsor TMF taxonomy for the site-owned document categories.
ICH E6(R2) Section 8
Phase 2
Document Collection and Filing
5File startup documents before enrollment
Protocol, investigator's brochure, regulatory and IRB approvals, and signed agreements filed before the first subject is enrolled at any site.
ICH E6(R2) Section 8.2
↓
6Collect and file documents continuously
Monitoring reports, updated approvals, and safety documentation filed as they are generated, rather than accumulated for later batch filing.
ICH E6(R2) Section 8.1
↓
7Version control superseded documents
Superseded versions of protocols, IBs, and other controlled documents retained and clearly marked, with the current version identifiable at all times.
TMF Reference Model
↓
Is the document complete, legible, and correctly filed?
YES → File and index NO → Return for correction
Phase 3
Quality Control and Completeness Review
8Conduct periodic TMF completeness reviews
Scheduled reviews assess the TMF against the essential document index, identifying missing, outdated, or misfiled documents.
ICH E6(R2) Section 8
↓
9Reconcile sponsor TMF against site files
Sponsor TMF and investigator site file compared to confirm both hold the required, consistent, and correctly versioned documentation.
TMF Reference Model
↓
10Resolve missing or misfiled document findings
Gaps identified during review tracked to resolution, with root cause addressed to prevent recurrence across other sites or documents.
ICH E6(R2) Section 8
Phase 4
Archival and Retention
11Conduct final TMF completeness check
Comprehensive review performed at trial closure confirming the TMF is complete before archival, with any final gaps resolved.
ICH E6(R2) Section 8
↓
12Migrate and lock the TMF for archival
TMF migrated to its archival system and locked against further edits, preserving the record exactly as it existed at trial completion.
EU CTR 536/2014
↓
13Retain the TMF per required retention period
TMF retained for the period required by applicable regulation, typically a minimum number of years following trial completion or product approval.

TMF Ownership and Structure

Trial Master File (Sponsor)

Sponsor-owned collection of trial-wide essential documents, held centrally regardless of how many sites or vendors are involved.

ICH E6(R2) Section 8

Investigator Site File

Site-owned subset of essential documents specific to that site's conduct of the trial, maintained by the investigator.

ICH E6(R2) Section 8

Combined and Vendor-Held TMF

Where a CRO or vendor holds part of the TMF, the sponsor retains ultimate responsibility for its completeness and accuracy.

ICH E6(R2) Section 5.2

Document Quality Standards (Overview)

Legibility Every filed document must be clearly readable, including scanned copies, with no illegible entries or cut-off pages.
Completeness Documents filed in full, including all pages, signatures, dates, and attachments referenced within the document itself.
ALCOA+ principles Filed records remain attributable, legible, contemporaneous, original, and accurate, consistent with data integrity expectations across the TMF.
Contemporaneous filing Documents filed at or near the time they are generated, avoiding the retrospective reconstruction risk of batch filing at closeout.

TMF Completeness Metrics (Overview)

Filing timeliness Measures the interval between document generation and filing, flagging documents filed outside the expected window.
Completeness percentage Tracks the proportion of expected documents present against the essential document index at a given point in the trial.
Quality issue rate Tracks documents returned for correction due to legibility, completeness, or misfiling issues, by site and by document type.
Reconciliation status Tracks open discrepancies between the sponsor TMF and investigator site files pending resolution.

TMF Management Quality Elements

TMF Reference Model Structure Filing taxonomy built on a recognized industry standard, enabling consistent structure across trials and easier vendor or CRO alignment.
Real-Time Filing Discipline Documents filed as they are generated rather than reconstructed retrospectively, supporting contemporaneous, defensible records.
Version Control Current and superseded versions of controlled documents clearly distinguished, with the current version always identifiable.
Access Control and Audit Trail Role-based access to the TMF with a complete audit trail of who filed, modified, or approved each document.
Periodic Completeness Reviews Scheduled reviews against the essential document index throughout the trial, not only at closeout.
Site-Sponsor Reconciliation Sponsor TMF and investigator site files periodically compared to confirm consistency and identify gaps early.
Archival Readiness TMF structured and quality-controlled throughout the trial so final archival requires confirmation, not last-minute assembly.
Retention Period Compliance TMF retained in an accessible, unaltered state for the full period required by applicable regulation across all relevant jurisdictions.

Never do this

Wait until trial closeout to assemble the TMF from scratch. File a document without confirming it is legible and complete. Allow superseded and current document versions to be indistinguishable. Destroy or migrate TMF records before the required retention period has elapsed.

TMF vs ISF

The Trial Master File is the sponsor's complete, trial-wide collection of essential documents. The Investigator Site File is the site-specific subset maintained by the investigator. Together they should be reconcilable into one complete record of the trial.

Real-time vs retrospective filing

Real-time filing captures documents as they are generated, supporting contemporaneous, defensible records. Retrospective filing reconstructs the TMF after the fact, increasing the risk of gaps, misdated entries, and inspection findings.
